Mesonephric-like adenocarcinoma in the endometrium:report of one case and review of literature

Abstract:Objective To discuss the clinical pathological features, diagnosis, differential diagnosis, and prognosis of metastatic-like adenocarcinoma (MLA) of the endometrium. Methods A case of endometrial MLA was collected and analyzed, and its clinical pathological characteristics, immunophenotype, and prognosis were retrospectively analyzed. Relevant literature was reviewed. Results The patient was a 57-year-old female who presented with vaginal bleeding 4 months after menopause. The tumor occurred in the uterine cavity. Under the microscope, the tumor showed glandular, sieve-like, tubulo-cystic, papillary, and solid arrangements. Some lumens contained eosinophilic gel-like secretions. Tumor cells were cuboidal, oval, shoe-string-like, and columnar, with eosinophilic cytoplasm, mild to moderate atypia, nuclear overlapping, nuclear grooves, and mitotic figures. Immunohistochemical staining showed diffuse strong positivity for GATA-3, and weak positivity for calretinin and CD10. TTF1, ER, and PR were negative. Conclusion MLA is a rare gynecological tumor recently recognized. It can occur in the uterus and ovary. Its clinical presentation is not specific, leading to frequent misdiagnosis and missed diagnosis. The diagnosis mainly depends on histopathology and immunophenotype.
