Abstract:Objective: To explore the clinical and pathological characteristics, diagnosis, and differential diagnosis of thymic differentiation thyroid carcinoma (CASTLE), and to conduct a review analysis of CASTLE, in order to provide references for further research on this disease. Methods: A retrospective analysis was conducted on the clinical manifestations, histopathological features, immunohistochemistry, diagnosis, and differential diagnosis of one case of pathologically confirmed thymic differentiation thyroid carcinoma, and a literature review was performed. Results: The patient was a 60-year-old female who was admitted to the hospital 8 days after discovering a thyroid nodule during a physical examination. A mass of approximately 52 mm × 46 mm × 31 mm was palpable in the left lobe of the thyroid. After undergoing "total thyroidectomy + left functional neck lymph node dissection + regional lymph node resection", the patient recovered well. Pathological examination showed a neoplastic lesion in the left lobe of the thyroid, with normal thymus tissue found in the surrounding tissues. Tumor cells were arranged in nests and cords, with lymphocytic infiltration in the stroma. Immunohistochemistry showed CD117(+), CD5(+), CK19(+), Ki-67(40%). Conclusion: CASTLE is a rare subtype of thyroid cancer, usually with a good prognosis. The diagnosis of this disease mainly relies on immunohistochemical results. Currently, radical surgical resection is the preferred treatment for CASTLE.
