Diphenotypic hepatocellular carcinoma:a clinicopathological analysis of 19 cases

Abstract:Objective To investigate the clinicopathological features and prognosis of dual-phenotype hepatocellular carcinoma(DPHCC).Methods 19 cases of DPHCC diagnosed in Xiang'an Hospital of Xiamen University and Xiamen Humanity Hospital from 2019 to 2024 were collected.HE staining and immunohistochemical staining were used to investigate the clinicopathological features of DPHCC.In addition,312 cases of common hepatocellular carcinoma diagnosed in the same period were used as the control group to compare the clinicopathological differences between them.Results The 19 cases of DPHCC included 15 males and 4 females.Histologically,the tumor cells were arranged in trabecular,pseudoglandular or solid pattern,with little fibrous stroma.The cells were heterotypic,cytoplasmic basophilia,and nucleoli were visible.Immunohistochemically,tumor cells were positive for Heppar-1(17/19),GPC3(18/19),ARG(14/19),AFP(15/19),CK19(16/19),CK7(15/19),MUC-1(10/19),EGFR(16/19).Compared with common hepatocellular carcinoma,DPHCC showed higher rate of vascular invasion and tumor progression(P<0.05),but there were no significant differences in age,sex,part,gross classification,histological classification,and degree of differentiation(P>0.05).Conclusions DPHCC is more aggressive in biological behavior with a poor prognosis.
