Clinicopathological features and tumor-associated immune microenvironment of duodenal-type follicular lymphoma

Abstract:Objective To investigate the clinicopathological features and tumor-related immune microenvironment of duodenal follicular lymphoma(DTFL).Methods The clinical features,morphological features,immunophenotypes,endoscopic findings and treatment of 7 cases of DTFL were retrospectively analyzed.The tumor-related immune microenvironment of DTFL was explored by using the GEO database and immunohistochemical methods.Results Most of the patients were female with a median age of 51 years.All the lesions occurred in the descending part of the duodenum.Endoscopic findings were mainly diffuse or flaky granular hyperplasia.Microscopically,the lesions showed nodular structure and diffuse distribution of lymphoid tissue.The tumor cells expressed B-lineage antigen and germinal center markers,and follicular dendritic cells mainly gathered around germinal centers.Some patients recovered spontaneously without treatment or had no progression of the lesion,and 2 patients were cured after rituximab treatment.A significant increase in resting NK cells and a significant decrease in resting mast cells were found in DTFL,as verified by the GEO database and immunohistochemical methods.Conclusion DTFL is morphologically and immunophenotypically similar to low grade follicular lymphoma in lymph nodes,but can be differentiated by the specific distribution of follicular dendritic cells.DTFL is an indolent lymphoma with a good prognosis and some patients can heal spontaneously.The tumor immune microenvironment may be related to resting NK cells and resting mast cells.
