Metastatic lung embryonal carcinoma:report of a case and review of literature

Abstract:Embryonal carcinoma is a rare type of germ cell tumor. This article reports a case of embryonal carcinoma presenting with pulmonary metastasis as the initial symptom. The patient was a 38-year-old male who was found to have multiple pulmonary masses on CT, and tested positive for HIV. After admission, a CT-guided percutaneous lung biopsy was performed, and the postoperative specimen was sent for pathological examination. Grossly, three gray-white strip-like tissues were observed, measuring 0.4–0.8 cm in length, with the largest diameter being 0.1 cm. Microscopically, the cancer cells showed solid structures, deeply basophilic cytoplasm, unclear cell membranes, large nuclei, crowded and overlapping, pleomorphic nuclei with a vesicular appearance, and single or multiple large nucleoli were visible. Numerous mitotic figures and apoptotic bodies were observed, and lymphocytic infiltration was present in the stroma. Immunohistochemical markers SALL4, OCT4, and CD30 were positive, while CD117 was negative. The diagnosis was pulmonary metastatic embryonal carcinoma.
