Application value of cell DNA quantitative analysis in differential diagnosis of pleural effusion

Abstract：Objective To explore the diagnostic effect of combined application of cell DNA quantitative analysis and liquid-based thin-layer cytology in the diagnosis of pleural effusion. Methods A retrospective analysis was conducted on 133 patients with suspected malignant pleural effusion who underwent liquid-based cytology at China-Japan Friendship Hospital from December 2022 to February 2024. All patients also underwent cell DNA quantitative analysis, and tissue samples were sent for histopathological examination. Using the histopathological results as the 'gold standard', the diagnostic accuracy of cell DNA quantitative analysis, liquid-based thin-layer cytology, and the combination of both methods in the diagnosis of pleural effusion was analyzed. Results Among the 133 cases of suspected malignant pleural effusion by liquid-based thin-layer cytology, 89 were finally diagnosed as malignant by pathological examination. The sensitivity of DNA quantitative analysis was 89.9% (80/89), and the specificity was 54.5% (24/44). The accuracy of liquid-based cytology was 66.9% (89/133). The accuracy of combined DNA quantitative analysis and liquid-based cytology was 78.2% (104/133), which was higher than that of single liquid-based cytology, with a statistically significant difference (P < 0.05). Conclusion The combined application of cell DNA quantitative analysis and liquid-based thin-layer cytology can complement each other, improving the diagnostic accuracy of malignant pleural effusion and providing valuable information for the diagnosis of suspiciously malignant pleural effusion.
