Metastases of clear cell renal cell carcinoma to the thyroid gland:report of one case and review of literature

Abstract:Objective To investigate the pathological characteristics, differential diagnosis, immunophenotype, and molecular pathological features of thyroid metastatic renal clear cell carcinoma. Methods A case of renal clear cell carcinoma with thyroid metastasis admitted to Shandong Provincial Third Hospital in 2022 was collected. Hematoxylin-eosin (HE) staining, immunohistochemical staining, and fluorescence in situ hybridization (FISH) were performed to analyze the histomorphological features, immunophenotype, molecular pathological characteristics, and diagnostic pitfalls. Combined with relevant literature, the key points of pathological diagnosis were summarized. Results A 71-year-old female patient presented with hematuria as the initial symptom. Imaging revealed a renal tumor and a thyroid tumor. The pathological result of the renal tumor was clear cell carcinoma. One month later, rapid pathology of the thyroid suggested metastatic renal clear cell carcinoma. Routine immunohistochemical staining showed that the tumor cells expressed PAX-8, CD10, CAIX (+), and Vimentin, while TG, TTF-1, and CK7 were negative. FISH analysis of chromosome 3p showed chromosomal abnormalities, characterized by a haploid change. Conclusion For patients with a history of renal cancer, the occurrence of thyroid metastasis should be highly suspected. If there is no history of renal cancer, a transparent cell tumor of the thyroid needs to be differentiated from primary transparent cell tumors of the thyroid and other metastatic transparent cell tumors.
