Abstract:Objective To explore the clinical pathological features, immunophenotype, diagnosis, and differential diagnosis of duodenal sarcomatoid carcinoma (SCA), and to review related literature. Methods A case of a 46-year-old female patient with a large ulcerative lesion in the duodenum was reported, and clinical and pathological data were collected. Results Under the microscope, the tumor nests showed extensive necrosis, with diffuse and sheet-like growth patterns. The tumor mainly consisted of two components: epithelioid cells and mesenchymal-like cells, which were mixed and distributed locally in a glandular pattern. Immunohistochemistry: CK-Pan(+), CK7(+), CK19(+), EMA(+), Vimentin(+), Ki-67(+30%), CK20(-), P53(-), S-100(-), HER2(0). Conclusion SCA is a rare tumor that exhibits both carcinomatous and sarcomatous morphological features. SCA occurring in the duodenum is extremely rare, with only 8 cases reported worldwide to date. Morphologically, SCA resembles carcinosarcoma and requires differentiation through immunohistochemistry.
