Intraoperative frozen misdiagnosis of ovarian metastatic cervical adenocarcinoma:report of a case and review of literature

Abstract:Objective: Ovarian secondary tumors tend to mimic primary ovarian surface epithelial tumors, making them prone to misdiagnosis. This study analyzes the reasons for misdiagnosis of a case of ovarian tumor as primary ovarian cancer during intraoperative frozen section, and proposes measures to prevent misdiagnosis. Methods: A retrospective analysis was conducted on one case of misdiagnosed intraoperative frozen section at Jiangyin Traditional Chinese Medicine Hospital in September 2022, and relevant literature was reviewed. Results: The patient presented with an ovarian tumor as the initial symptom, and the intraoperative frozen section pathology diagnosis mistakenly identified it as primary ovarian endometrioid carcinoma. After a radical hysterectomy, an occult cervical tumor was found. The two tumors had similar morphological features, and both showed ER(-), PR(-), p16 nuclear and cytoplasmic diffuse positivity by immunohistochemistry. Conclusion: Some cervical adenocarcinomas have an insidious onset, and patients often present with metastatic tumors from other organs as the initial symptom. Summarizing diagnostic experience and enhancing the awareness of pathologists and clinicians about this disease can help avoid misdiagnosis and mistreatment.
