Secretory carcinoma of external auditory canal skin with retroauricular lymph node metastasis:a clinicopathological analysis and review of literature

Abstract:Objective To explore the clinical and pathological features, diagnosis, and differential diagnosis of primary cutaneous secretory carcinoma (PSCS) with lymph node metastasis. Methods Clinical and pathological data of a patient with PSCS accompanied by lymph node metastasis were collected, and relevant literature was reviewed. Results The patient was a 40-year-old male who had discovered a mass in the right external auditory canal for half a year. The gross specimen of the right external auditory canal mass was fragmented tissue, and one right postauricular lymph node was also observed. Microscopically, the tumor cells in both specimens showed microcystic, glandular/tubular, sieve-like, and papillary structures, with pale eosinophilic secretions in the cysts and glandular lumens, and visible mitotic figures and prominent nucleoli; both specimens showed diffuse positive expression of CK7, S-100, vimentin, and Mammaglobin, with a low Ki67 index; both detected ETV6-NTRK3 gene rearrangements. Conclusion PSCS with lymph node metastasis is extremely rare, with specific molecular genetic characteristics. A comprehensive analysis combining clinical data, microscopic morphology, immunohistochemistry, and molecular genetics is required for diagnosis.
