Abstract:Objective: To discuss the clinical and pathological characteristics, immunohistochemical phenotype, and differential diagnosis of this rare primary cavernous hemangioma of the thyroid. Methods: A retrospective analysis was conducted on a case of primary cavernous hemangioma of the thyroid treated at Chengdu Fifth People's Hospital in November 2022, along with a review of relevant literature. Results: The tumor under the microscope consisted of dilated, thin-walled vascular spaces that were clearly demarcated from the surrounding thyroid tissue. The vascular spaces varied in size and were irregular, with endothelial cells lining the walls. Lymphocytes and eosinophilic secretions were present within the vascular spaces, and some stroma showed aggregates of lymphocytes. Immunohistochemical staining showed that the endothelial cells were positive for CD31, CD34, and ERG, and negative for D2-40, with a very low Ki-67 index. Based on the morphological and immunohistochemical findings, the diagnosis of cavernous hemangioma was supported. Conclusion: Primary cavernous hemangioma of the thyroid is rare, and preoperative diagnosis presents certain challenges. Postoperative diagnosis requires comprehensive evaluation based on histomorphology and immunohistochemistry.
