Tall cell carcinoma with reversed polarity:report of a case

Abstract:Objective: To investigate the clinical and pathological characteristics of high-cellularity carcinoma of the breast with inverted cell polarity (TCCRP). Methods: A retrospective analysis was conducted on the clinical and pathological features of one case of high-cellularity carcinoma of the breast with inverted cell polarity. Results: The patient was a 50-year-old female who presented with an unintentional left breast mass for 30 days. Breast ultrasound showed a 2.3 cm × 1.6 cm × 1.5 cm hypoechoic nodule in the upper inner quadrant of the left breast, with unclear boundaries, irregular shape, and rich blood flow signals, consistent with BI-RADS 4C category. Cytology of fine needle aspiration showed a few atypical epithelial cells, suggesting cancer cells. Histological examination revealed solid nest-like and infiltrative growth patterns, with tall columnar tumor cells showing mild to moderate atypia and inverted nuclear polarity. Immunohistochemistry: Tumor cells were positive for GATA-3 (+), E-cadherin (+), IDH2R172S (+), CK5/6 (+), CK7 (+), and CK14 (+), while ER, PR, HER2, p63, TTF-1, Calponin, Tg, and PAX-8 were all negative. Conclusion: High-cellularity carcinoma of the breast with inverted cell polarity is rare, and diagnosis requires combination of histopathological morphology and immunohistochemical staining. Clinical pathologists should fully recognize this tumor to avoid misdiagnosis.
