Abstract:Objective: To investigate the clinical and pathological characteristics, immunophenotype, differential diagnosis, and prognosis of ovarian clear cell borderline tumor (CCBT). Methods: A retrospective analysis was conducted on the clinical and pathological features, immunophenotype, molecular characteristics, differential diagnosis, and prognosis of one case of ovarian CCBT treated at Hefei Maternal and Child Health Hospital, and relevant literature was reviewed. Results: The mass showed a mixed cystic-solid appearance with a diameter of approximately 8 cm. The solid area had a gray-white, honeycomb-like, firm cut surface. Under the microscope, the solid area had a fibroma-like stroma with glands of varying sizes and uneven distribution. Most glandular lumens were lined by a single layer of epithelium. The tumor cells were flat, cuboidal, or boot-shaped, with some cells showing clear cytoplasm. The cells exhibited mild to moderate atypia, rare mitotic figures, no cohesive growth patterns (such as sieve-like, solid, microcystic, or papillary structures), and no stromal infiltration. Conclusion: Ovarian CCBT is a rare ovarian borderline tumor. The diagnosis mainly relies on morphological features and immunophenotypic characteristics. The combined use of immunohistochemical markers NapsinA and HNF-1β can help differentiate it from other types of ovarian epithelial tumors. The prognosis of isolated ovarian CCBT is good, with no recurrence cases reported. Long-term follow-up is recommended.
