Abstract:Objective To investigate the clinical and pathological features of classical Hodgkin lymphoma presenting with a thyroid nodule as the initial manifestation. Methods A case of classical Hodgkin lymphoma with a thyroid nodule as the first symptom was reported, and its clinical and pathological characteristics were analyzed along with a review of the literature. Results The patient was a 22-year-old female who was admitted to the hospital due to rapid enlargement of a neck mass for more than two months. She underwent subtotal thyroidectomy. Microscopically, the tumor tissue showed diffuse infiltration growth, with diagnostic mononuclear, binucleated, and multinucleated R-S-like cells exhibiting prominent eosinophilic large nucleoli. Immunohistochemical and in situ hybridization results showed that the tumor cells were negative for LCA, weakly positive for PAX-5 and PAX-8, strongly positive for CD30 and MUM1, and positive for EBERs in situ hybridization. Conclusion Classical Hodgkin lymphoma presenting with a thyroid nodule as the initial manifestation is rare clinically. Diagnosis requires combination of histopathological morphology and immunohistochemical staining. Clinicians and pathologists should be alert to the possibility of lymphoma when a young female presents with a rapidly growing thyroid mass to avoid misdiagnosis.
