Gastric SMARCA4-deificient undifferentiated carcinoma:report of a case and review of literature

Abstract:Objective To investigate the clinical and pathological features of gastric SMARCA4-deficient undifferentiated carcinoma (SMARCA4-dUT). Methods A case of gastric SMARCA4-dUT was collected, and clinical and pathological data were analyzed. Hematoxylin-eosin staining, immunohistochemistry, and molecular testing were performed to summarize the histological features, immunophenotype, and molecular characteristics of the tumor, and relevant literature was reviewed. Results Under the microscope, the tumor cells showed a diffuse nest-like or sheet-like distribution with poor adhesion. Local areas showed rhabdoid cells with vacuolar nuclei, prominent nucleoli, and frequent mitotic figures. Necrosis was present in some regions. Immunohistochemistry showed complete loss of SMARCA4 (BRG1) expression, while SMARCB1 (INI1) and SMARCA2 were positive. CK and Syn were focally positive, and CgA was negative. Molecular testing revealed SMARCA4 gene mutation. Conclusion Gastric SMARCA4-dUT is a rare gastric malignant tumor with poor differentiation, which is easily misdiagnosed. Familiarity with its pathological features and immunohistochemical detection can help in accurate diagnosis and treatment.
