Superficial spread of the cervical high-grade squamous intraepithelial lesions involving the endometrium:report of a case and review of literature

Abstract:Objective: To investigate the clinical and pathological features, possible pathogenesis, treatment, and prognosis of cervical high-grade squamous intraepithelial lesion (HSIL) with superficial spread involving the endometrium. Methods: A case of postmenopausal elderly female with cervical HSIL with superficial spread involving the endometrium was reported, and relevant literature was reviewed. Results: Postoperative pathology of total hysterectomy showed cervical HSIL with focal microinvasion (<3 mm), and the cervical lesion continued to the junction of the lower uterine segment and cervix. It spread "carpet-like" on the surface to the endometrium, involving its surface and glands, without infiltration into the stroma or myometrium of the uterine body. Both cervical and endometrial lesions expressed squamous epithelium markers p40 and showed diffuse expression of p16, but the endometrial lesion was negative for ER and PR. Conclusion: Cervical HSIL can directly spread upward into the uterine cavity; pathologists should enhance their awareness of such lesions.
