Clinicopathological featuers of primary SMARCA4-deficient gallbladder carcinoma

Abstract:Objective To analyze the clinical and pathological characteristics of primary SMARCA4-deficient cholangiocarcinoma. Methods Clinical data of one patient with primary SMARCA4-deficient cholangiocarcinoma were collected, and the clinical and pathological features were analyzed by HE staining and immunohistochemistry. Results Under the microscope, the tumor cells were arranged in a diffuse sheet-like pattern, mainly spindle-shaped and oval cells, some showing epithelioid morphology, some sarcomatoid, with a few cells losing adhesion, nuclei located at the periphery with rhabdoid appearance, obvious nuclear atypia, frequent pathological mitotic figures, and a small amount of adenocarcinoma mixed in. Immunohistochemistry: Tumor cells expressed CK, CK7, EMA, CAM5.2, and INI-1, but did not express Brg-1, MyoD1, S100, Desmin, or CD34. Conclusion Primary SMARCA4-deficient cholangiocarcinoma is rare, with diverse pathological morphology and poor prognosis, and is easily misdiagnosed. Timely and accurate diagnosis has important guiding significance for the treatment and prognosis of patients.
