Pancreatic mucinous cystic neoplasm:a clincicopathological analysis of 19 cases with review of literature

Abstract:Objective To investigate the clinicopathological features of mucinous cystic neoplasm(MCN)of the pancreas.Methods 19 cases diagnosed MCN after surgery were collected.Clinicopathological features,immunohistochemical phenotypes and molecular genetic changes were analyzed,and the relevant literatures were reviewed.Results 14 cases were MCNs with low-grade dysplasia,1 case with high-grade dysplasia,and 4 cases were MCN with invasive carcinoma.The ratio of man to women was 1 ∶ 18.13 cases were occurred in the body and tail of the pancreas,6 cases in the head.All malignant cases presented cyst with thickened wall,mural nodules or intracystic necrosis macroscopically,with invasive components of ductal adenocarcinoma,adenosquamous carcinoma,and tubulopapillary adenocarcinoma microscopically.All cysts of cases of MCN with dysplasia were smooth,consisted of cuboidal or columnar mucin-producing epithelium and spindle ovarian type stroma.Immunohistochemically,positive for CK7,MUC5AC,MUC6 were in all cases,SMAD4 expressed in all cases without loss,and positive for ER,PR in 9 and 11 ovarian type stroma,respectively.2 cases of MCN with invasive carcinoma were detected point mutation in codon 12 of exon2 in Kras gene.Condusion MCN is a very rare tumor arise from the endocrine portion of the pancreas.The majority of MCNs occurs in the pancreatic body and tail and most in female.Surgery is preferred for MCNs.The prognosis of MCN with invasive carcinoma is better than that of non-MCN-associated ductal adenocarcinoma.
