Gastric-type adenocarcinoma:a clinicopathological analysis of 15 cases

Abstract:Objective To investigate the clinicopathological features of the gastric-type adenocarcinoma(GAS)in order to improve the understanding of this disease.Methods The clinical manifestations,histological features and immunohistochemical phenotypes of 15 cases of GAS were analyzed and the related literature was reviewed.Results The average age of GAS was 51 years(34-74 years).Vaginal drainage and bleeding were the most common clinical manifestations,HPV tests were mostly negative,and the proportion of tumor marker CA199 was higher than that of CA125.The tumor was mostly located in the cervical canal,the lesion was not obvious.Histopathologically,GAS forms were diverse,mostly with diffuse infiltration of adenoid structures accompanied by significant pro-fibrous interstitial or inflammatory cell infiltration.The cell morphology spectrum was wide,with moderate to severe atypia mixed with mildly atypia epithelium.The positive rates of MUC6,MUC5AC,Villin,PAX8 and CK7 were high,while of ER,PR,P16 and those SATB2 were low.13 cases had tumor infiltration in the deep interstitial layer of the cervix,8 cases had cancer thrombus in the vasculature,8 cases had lymph node metastasis,and 8 cases had other involvement.Conclusion GAS is rare,prone to invasion and metastasis,poor prognosis,lack of specificity in clinical manifestations,latent lesions,diverse histopathological morphology,and easy to be missed and misdiagnosed.Accurate diagnosis is of great significance to patients,and it is necessary to combine clinical manifestations,fully understand the diversity of histological morphology and combined application of immunohistochemistry.
