High-grade follicular cell-derived thyroid carcinoma:a clinicopathological analysis of twelve cases

Abstract:Objective To investigate the clinicopathological characteristics,diagnosis and differential diagnosis of high-grade follicular cell-derived thyroid carcinoma and to improve the recogniton.Methods A total of 12 patients with high-grade follicular cell-derived thyroid carcinoma admitted from September 2019 to February 2023 were identified at the First Affiliated Hospital of USTC.The clinical features,histological morphology and immunohistochemical staining were analyzed,and relevant literatures were reviewed.Results There were 12 cases,including 3 males and 9 females,with a mean age of 54 years.Among them,8 cases were diagnosed as a cervical mass due to physical examination or self discovery,3 cases were recurrent cases,and 1 case was admitted due to chest wall pain.7 cases were poorly differentiated thyroid carcinoma(Turin criteria),and 5 cases were differentiated high-grade thyroid carcinoma.Microscopically,poorly differentiated thyroid carcinoma showed solid,trabecular and island patterns,with distorted and deeply stained nuclei.Among the differentiated high-grade thyroid carcinoma,4 cases were high-grade papillary thyroid carcinoma and 1 case was high-grade follicular thyroid carcinoma.The mitotic figures were ≥5/2 mm2.Immunohistochemistry showed that TTF-1 and PAX-8 were expressed,BRAF V600E was expressed in 3 cases,and Ki-67 proliferation index was 10%~70%.Follow-up showed that 5 cases had recurrence and 2 cases had metastasis.Conclusion High-grade follicular cell-derived thyroid carcinoma is a new type of endocrine tumor in the 2022 edition of WHO classification,and its prognosis is between well differentiated follicular cell-derived carcinoma and anaplastic carcinoma.Because of the low incidence and insufficient understanding at present,it has great significance to master its histopathological diagnostic criteria.
