Diagnostic value of core needle puncture biopsy and fine needle aspiration biopsy in malignant thyroid lesions

Abstract:Objective To determine the clinical value of core needle aspiration biopsy(CNB)and fine needle aspiration biopsy(FNA)in the diagnosis of benign and malignant thyroid lesions.Methods A total of 559 patients with thyroid diseases(including 498 cases for CNB and 558 cases for FNA)who underwent ultrasound-guided CNB or(and)FNA examination and subsequent surgical treatment were retrospectively collected.The sensitivity,specificity and accuracy of CNB and FNA diagnostic method were evaluated by comparing the biopsy and surgical pathological result,and the differences in tumor size,puncture times and smear number between the tumorous diagnostic group and the none tumorous group were compared.Results The diagnostic accuracy of CNB and FNA were 91.8%and 94.1%respectively;The sensitivity of CNB was significantly lower than that of FNA(P=0.027),but there was no significant difference in specificity(P=1.000);For CNB,there was significant difference in tumor size between the tumorous diagnosed group and the none tumorous group(P=0.002),but there was no significant difference in puncture times(P=0.628);While for FNA,there was no significant difference in tumor size,puncture times and smear numbers between the correctly diagnosed group and the misdiagnosed group(P>0.05).Conclusion FNA is more sensitive than CNB in the diagnosis of thyroid diseases;The larger the lesion size,the higher diagnostic accuracy of CNB.
