Elastofibroma dorsi:a clinicopathological analysis of 44 cases

Abstract:Objective To summarize the clinicopathological features of elastofibroma dorsi(ED).Methods A total of 44 cases of ED were retrieved,immunohistochemical staining,elastic fiber staining and analysis of the clinicopathological characteristics were performed.Results The average age of the patients was 60.1 years(ranged from 45 to 84 years),with 37 females and 7 males.Subscapular location was constant.31 cases(70.5%)were unilateral and 13 cases(29.5%)were bilateral.Patients complained of lump(44/44),pain(14/44),functional limitation(6/44)and feeling of foreign body(5/44).Macroscopically,all cases appeared as an irregular,pooly defined and unencapsulated mass.Cut section revealed solid and gray-white to gray-yellow surface with cystic degeneration in one case(2.3%).Microscopically,the tumor was constituted of collagen fibers and numerous beadlike,serrated or spherical elastic fibers,intermixed with islets of fatty tissue and blood vessels.Immunohistochemically,all cases were positive for Vimentin but negative for SMA.Desmin and S-100.Elastic fiber staining highlighted the elastic fibers.Surgical resection was done in 44 cases(52 sites).Complications were noted in 18 cases(40.9%),seroma in 14 cases(31.8%).No recurrence was noted during the follow-up period(3 months to 90 months,median 20.35 months).Conclusion Elastofibroma dorsi is an uncommon elastic fibrous tumor-like lesion.Typical location,histopathological features and elastic fiber staining are helpful for accurate diagnosis.Contralateral lesions should be carefully examined because of the possibility of missed diagnosis.Favorable clinical result can be obtained with surgical resection,and postoperative complications should be paid more attention.
