Paget's disease of the esophagus: clinical and pathological analyses

Abstract:Objective To investigate the clinicopathologic features and differential diagnosis of Paget's disease of the esophagus. Methods The clinical data of one case of Paget's disease of the esophagus were collected, its pathological and immunohistochemical features were analyzed, the tumor's pathological diagnosis and differential diagnosis were discussed based on related literature review. Results The patient was 73 years old male and had a clinical manifestation of dysphagia for two months. Microscopically, squamous epithelium in the middle and basement of the esophagus was infiltrated by large cells which were arranged in clusters, single or scatter. Tumor cells multifocally invaded into the mucosal and submucosal layers where they formed tubular nests, but there was no invasive growth beyond the basement membrane. Tumor cells were large, round and contained large nucleoli and ample, clear, or pale-staining cytoplasm, and had an increased nuclear cytoplasmic ratio, the nuclei were large, round, irregular, with coarse chromatin, and high mitosis. Immunohistochemical results showed the tumor cells were immunoreactive to CEA, EMA, CK7 and CK8/18. None were immune-reactive to CK20, CK14, P63, CK5/6, HMB45, Malan-A and S-100. Conclusion Esophageal Paget's disease is a very rare tumor, and differential diagnosis of it includes squamous cell carcinoma in situ, superficial malignant melanoma, adenocarcinoma, Pagetoid Bowen disease, lymphoma etc. Well awareness of the disease helps reduce misdiagnosis.
