Central giant cell lesion of the temporal bone : a clinicopathologic analysis of 15 cases

Abstract:Objective To study the clinicopathologic features,diagnosis and differential diagnosis,the pathogenesis of central giant cell lesion of the temporal bone.Methods Fifteen cases of central giant cell lesion of the temporal bone from the Chinese PLA General Hospital were investigated by clinical findings,histopathology and immunohistochemistry,related literatures were also reviewed,and all cases were followed up.Results Twelve patients first presented ear symptoms.According to imaging and intraoperative findings,the lesions of nine cases presented destructive growth,5 cases were limited with clear boundaries.One case was unknown.Microscopic examination revealed that the lesion was composed of the spindle-shaped fibroblast cells or myofibroblast cells.Hemorrhage and hemosiderincould be observed at the most cases,accompanied by infiltration of macrophages,lymphocytes and neutrophils.Osteoclast-like giant cells appeared in the hemorrhagic region and evenly distributed or clustered.Cholesteatoma was observed in one case.Three cases had locally recurrent postoperative 11 months,23 months,36 months.4 cases were lost to follow-up.Others cases had remained free of locally recurrent or metastatic disease through 18 months to 120 months of follow-up.Conclusions Central giant cell lesion is a non-neoplastic aggression lesion.Central giant cell lesion of the temporal bone is often accompanied with tympanitis.The cases with extensive lesions and destruction of surrounding tissue are more likely to recur.
