Malignant solitary fibrous tumor outside the pleura : a clinicopathological study of two cases

Abstract:Objective To investigate the clinical pathological features,diagnosis,differential diagnosis and prognosis of malignant solitary fibrous tumor (SFT) outside the pleura.Methods Two cases of malignant SFT outside the pleura were retrospectively analyzed,and the relevant literature were reviewed.Results Two patients were male,aged 59 or 76 years respectively;one case was located in the right cerebral falx,another was located in the right shoulder.The tumor size was 3 cm × 3 cm × 2 cm and 8 cm × 6 cm × 3 cm,respectively.The microscopic tumor tissue morphology showed that striate,weaving,spiral structure,scattering with collagen in distribution.Spindle tumor cell was dominant,and the nuclei was short spindle type or ovoid;cell atypia in the cerebral falx was not obvious,but the tumor cell atypia with giant nucleus or coarse chromatin occuring in the shoulder was very obvious;mitosis was easy to see.The immunostaining of vimentin,CD34,STAT6,bcl-2 and CD99 was positive,but S-100,desmin,EMA and actin were negative.Conclusion Malignant SFT occuring in the cerebral falx or shoulder soft tissue is very rare,easily confused with other soft tissue tumors,such as meningioma,peripheral nerve sheath tumor,pleomorphic liposarcoma,synovial sarcoma.The confirmed diagnosis relies on pathological morphology and immunophenotyping.
