Intrapancreatic accessory spleen: a clinicopathological analysis of 4 cases

Abstract:Objective To analyze the clinical characteristics and morphology of intrapancreatic accessory spleen (IPAS).Methods The clinical data of 4 cases of IPAS were retrospectively analyzed.Results Patients' age ranged from 39 to 69 years with mean age of 52,including three males and one females.2 patients were suffered from dyspepsia,one patient had elevated CA19-9 level and one was asymptom.The CT scan and PET-CT of the pancreas demonstrated that a solid mass in the pancreatic tail was thought to represent pancreatic neuroendocrine tumor (PNET).Macroscopy revealed that encapulated mass was in pancreas tissue with a clear boundary,the cut surface was the same to spleen tissue.Microscopic examination demonstrated spleen tissue with white and red pulp.Conclusions IPAS is rare comparatively.Radiographically,these lesions are indistinguishable from pancreatic tumor,especially PNET,and the dignosis depends on pathology.A clear boundary is between IPAS and pancreas tissue;meanwhile,the morphology of IPAS is same as spleen tissue.In some extremely rare cases,epidermal cyst can present in IPAS.Fine needle aspiration (FNA) plays an important role in preoperative diagnosis.Without any symptom,the patients with IPAS or ECIPAS do not need surgery.
