Meningeal solitary fibrous tumours/haemangiopericytomas: a report of 18 cases and review of literature

Abstract:Objective To study the clinicopathological features,histological classification and differential diagnosis of meningeal solitary fibrous tumours/haemangiopericytomas(SFT/HPC).Methods 18 cases of SFT/HPCs were analyzed on clinical manifestations,MR and histomorphological characteristics.Results In the 18 cases,10 males and 7 females were included,aged 18 to 72 years,with median age of 44.4 years.Headache was the most common presenting symptom.The MR images of SFT/HPCs showed solitary,irregularly contoured masses,often dural-based,dural tail was observed at the periphery of the lesion.The histopathologic feature of SFT/HPCs was mesenchymal tumour of fibroblastic type,often showing rich branching vascular pattern,encompassing a histological spectrum of solitary fibrous tumour and haemangiopericytoma.The WHO classification of SFT/CNS was a three grading scheme,and every grade of tumours had its own feature.The NAB2-STAT6 FUSION led to nuclear high specificity and sensitivity of positive for STAT6 of SFT/HPCs,and the tumor cells were typically positive for CD34,vimentin and Bcl-2.Prominent reticulin staining surrounding individual or small clusters of cells were also observed.Conclusions In addition to routine imaging and histopathological examination,immunohistochemical study is essential to differentiate SFT/HPC from other differential diagnosis,for example,meningioma.Radical surgical resection followed by radiation therapy can be considered as the optimal management for this tumour.As postoperative recurrence seems unavoidable,long-term follow-up with serial imaging should be considered in all cases.
