Quantitative pathologic evaluation of neoadjuvant chemotherapy for breast cancer

Abstract:Objectives To establish a quantitative pathologic system for the chemotherapy response by computer-assisted image analysis.Methods Biopsy specimens and subsequent matched resection specimens were assessed in 277 patients with invasive breast carcinoma including 203 patients who received neoadjuvant chemotherapy (treated group) and 74 patients who received surgical management only (control group).By using a digital microscopic system and computer analysis software,the tumor cellularity in the cohort was measured.Results In the control group,tumors in the needle biopsies revealed significantly lower level of tumor cellularity than that in the resected tumors (P < 0.01).However,the tumor cellularity of core needle biopsy was positively correlated with resection (P < 0.01).Univariate survival analysis showed that patients with low residual tumor cellularity ratio had a more favorable OS (P < 0.05) and DFS (P < 0.05).And patients with lower product of residual tumor size ratio and residual cellularity ratio (PRR) also had a more favorable DFS (P < 0.05).Conclusions Our quantitative evaluation further refines the assessment of neoadjuvant chemotherapy response and may probably provide a supplement for current methods.
