Morphological observation of gliosarcoma : five cases report and review of literature

Abstract:Objective To investigate pathological and immunohistochemical features,diagnosis and differential diagnosis of gliosarcoma of cerebral hemispheres.Methods Morphological observation,histochemistry and immunohistochemistry were applied in five cases of gliosarcoma from January 2008 to October 2016.Results Four cases were female and one was male with mean age of 55 years.Clinical symptoms included changes of behavior,dizziness,headache,nausea/ vomiting,and so on.MRI showed the solid-cystic masses of equal signal in T2 and T1 weighted image,with edema around and circular or abnormal heterogeneous enhancement.Morphologically,the sarcoid components were principal with fibrosarcoma in four cases and bone differentiation in one case.The glial area with high histological grade spread in sarcoma component patchilly.Immunohistochemically,Ki-67 index was about 10%~ 60%,GFAP and Oligo-2 were positive in glial components,and IDH-1 was negative in five cases.P16 and EGFR were positive in three cases,while MGMT was positive in one case.Reticulin staining was positive in the sarcoid areas.Conclusion Gliosarcoma is a variant of IDH-wildtype of glioblastoma,characterized by a biphasic tissue pattern with alternating areas displaying glial and mesenchymal differentiation.The prognosis and treatment are similar with that of glioblastoma.
