CD5 and CD23 negative chronic lymphocytic leukemia: report of a case and review of literature

Abstract:Objective To explore the clinicopathologic features and diagnosis and differential diagnosis of CD5 and CD23 negative chronic lymphocytic leukemia.Methods One case of the leukemia was reported.The clinical data,pathological and immunohisochemical features of this disease were reviewed and discussed with the relevant literature.Results Lymph node biopsy showed abundant small mature lymphocytes and few pseudofollicular proliferation centers composed of prolymphocytes and paraimmunoblasts.The tumor cells were positive for CD20 and bcl-2,and negative for CD5,CD23,CD10,BCL6,MUM1,cyclin D1 and TdT.About 5% of the neoplastic cells showed immunoreactivity for Ki67.Bone marrow biopsy showed patchy infiltration of small lymphocytes;the tumor cells were positive for CD20 and BCL2,and negative for CD43,CD5,CD23,cyclin D1,CD10 and bcl-6.Fluorescence in situ hybridization (FISH)of the neoplastic cells suggested del(13q14),del(13q14.3)and trisomy 12.Conclusion CD5 and CD23 negative chronic lymphocytic leukemia is rare,and its differential diagnosis is necessary from other B cell chronic lymphoproligerative disorders,especially nodal marginal zone B cell lymphoma and lymphoplasmacytic lymphoma.
