Clinicopathologic features of complex sclerosing lesion in different thyroid diseases

Abstract:Objective To investigate the clinicopathologic characteristics of complex sclerosing lesion in different surgical resection of thyroid diseases and significance in differential diagnosis of thyroid diseases.Methods A total of 971 cases of surgically-removed thyroid lesions encountered during the period of 2012 to 2015 were enrolled.103 cases with obvious fibrosis were detected,including 67 cases of complex sclerosing lesion.Clinical pathological features were analyzed and the expression of CK19,TPO,TG,and Ki-67 was studied by means of immunohistochemnistry.Results 24 cases of papillary thyroid carcinoma (35.8%),26 cases of papillary microcarcinoma (38.8%),1 case of thyroid adenoma (1.5%),10 cases of nodular goiter (14.9%),3 cases of Hashimoto thyroiditis (4.5%),2 cases of multifocal fibrosing thyroiditis(3%)and 1 case of granulomatous thyroiditis (1.5%) were discovered in the complex selerosing lesions.There was different intensity of fibrosis in different thyroid lesions,assumed a stellate configuration.In different thyroid diseases,the number and spatial distribution of the fibrotic foci,the nuclear characteristics and the features of the surounding follicles were different.In papillary carcinomas,CK19 and TG were positive,TPO was negative and Ki-67 labeling index was below 15%.However,in other thyroid diseases with complex sclerosing,CK19 was negative,both TG and TPO were positive,and Ki-67 labeling index was below 5%.Conclusions Complex sclerosing lesion could occur in different thyroid lesions,which is not the unique histologic feature of papillary carcinomas.More careful observation could be needed to avoid false diagnosis.
