Malignant phyhodes tumors: clinicopathologic and prognostic analyses of 8 cases

Abstract:Objective To study the clinicopathologic features,immunophenotype,differential diadnosis and prognosis of malignant phyllodes tumors (PT).Methods Eight patients with malignant PT from 2009 to 2016 were selected for clinicopathologic and immunohistochemical analysis.These data were correlated with the follow-up results.Results The age of the patients ranged from 15 to 51 years.The mean tumor size was 5.5 cm (1.0-8.0 cm).Histologically,PT had stromal overgrowth,with sarcomatous differentiation with leaf-like projections.Three cases had heterologous elements.The mean positive index of Ki-67 was 33.5%.The stromal tumor cells were positive for CD34 and p16 in 1/8 and 7/8 cases.But,ER,PR and Her2 were negative.At the end of the follow-up period (4 to 40 months),there were tumor recurrences in 4/8 cases.Metastasis occurred in 2/8 patients,and one died of the tumors.Conclusions Malignant PT is a rare neoplasm and carries an indolent behavior.Tumors with heterologous elements show poor prognosis.Immunohistochemical analysis is a auxiliary tool in confirming the diagnosis.
