Clinicopathological features of inflammatory pseudotumor-like follicular dendritic cell tumor

Abstract:Objective To explore the clinicopathologic features of inflammatory pseudotumor-like follicular dendritic cell tumor (IPT-like FDCT).Methods The clinical data,pathological features,immunohistochemical staining and in situ hybridization for Epstein-Barr virus (EBV)-encoded RNA (EBER) findings of five cases of IPT-like FDCT were reported.Results Among the 5 cases diagnosed as IPT-like FDCT,two cases were female,three cases were male,aged from 35 to 78 years(mean 52.6 years).The neoplasms occurred in the spleen (three cases),liver (one case) and retroperitoneal (one case).The neoplasms showed well-defined borders.Histological examination showed tumor cells admixed with abundant lymphocytes and plasma cells in the background arranging in vague fascicles or dispersal arrays.The nuclei were elongated or oval in shape,with vesicular chromatin,and distinct,often central nucleoli.The nuclei were mild atypia.Immunohistochemically,the tumor cells were positive for CD21,CD35 and clusterin and also positive for EBER in situ hybridization.It was noteworthy that 3 cases accompanied by high expression of IgG4 plasma cells.All cases went complete surgical excision of tumors and were followed up for 2 ~ 96 months,with 4 patients disease-free and 1 patient lost of followup.Conclusions IPT-like FDCT is a rare low-grade malignant tumor and easily misdiagnosed.The diagnosis and differential diagnosis of IPT-like FDCT mainly relies on histopathology,immunohistochemistry and EBER in situ hybridization.
