Effect of carbon dioxide pneumoperitoneum management on relieving postoperative shoulder pain in patients undergoing gynecological laparoscopic surgery

Abstract:Objective To explore the effect of a refined carbon dioxide pneumoperitoneum management protocol on the occurrence and severity of postoperative shoulder pain in patients undergoing gynecological laparoscopic surgery.Methods A total of 100 patients who underwent laparoscopic surgery in the Department of Gynecology of the People's Hospital of Hechi from 2022 to 2024 were selected.Using a random number table method,patients were allocated into an observation group and a control group,with 50 patients in each group.The control group received conventional carbon dioxide pneumoperitoneum management during surgery,while the observation group underwent a refined pneumoperitoneum management protocol,including controlled low-pressure pneumoperitoneum,active gas evacuation upon procedure completion,and postural intervention.Outcome measures compared between the two groups included shoulder pain Visual Analogue Scale(VAS)scores at 6,24,and 48 h postoperatively,incidence of postoperative shoulder pain,time to first ambulation,length of hospital stay,rate of additional analgesic administration,and patient satisfaction.Results After intervention,the VAS scores for shoulder pain in the observation group were significantly lower than those in the control group at all time points(P<0.05).The overall incidence of postoperative shoulder pain was 24.00%in the observation group,significantly lower than 56.00%in the control group(P<0.05).Within 24 h after surgery,the rate of additional analgesic administration in the observation group(7 cases,14.00%)was significantly lower than that in the control group(17 cases,34.00%).No statistically significant difference was observed in the incidence of postoperative nausea and vomiting between the two groups.Furthermore,patient satisfaction scores regarding postoperative pain management were significantly higher in the observation group than in the control group(P<0.05).Conclusion Implementing refined carbon dioxide pneumoperitoneum management in patients undergoing gynecological laparoscopic surgery can effectively reduce the severity and incidence of postoperative shoulder pain,promote early patient recovery,and shorten the length of hospital stay,which has significant clinical promotion and application value.
