Risk factors for recurrence in postoperative radiotherapy patients with breast cancer and their relationship with different radiotherapy doses

Abstract:Objective To investigate the risk factors for recurrence in postoperative radiotherapy patients with breast cancer and analyze the relationship between different radiotherapy doses and recurrence risk,providing a basis for individualized radiotherapy planning.Methods A retrospective analysis was conducted on 100 patients with stage II-III breast cancer who received postoperative radiotherapy from April 2022 to April 2024.The patients were divided into a control group(conventional dose of 50 Gy)and an observation group(dose adjusted based on pathological features,with tumor bed boost to 60 Gy or supraclavicular region dose of 54 Gy).The recurrence rates were compared between the two groups,and univariate and multivariate Logistic regression analyses were performed to identify risk factors for recurrence.Results The observation group showed significantly lower recurrence rates and distant metastasis rates compared to the control group(P<0.05).Multivariate analysis identified age<40 years,tumor diameter≥5 cm,lymph node metastasis,PR negativity,clinical stage III,and unadjusted radiotherapy dose as independent risk factors(OR 2.965-6.614).Conclusion Individualized radiotherapy dose adjustment based on pathological features can reduce recurrence risk in high-risk patients,particularly through strategies of tumor bed boost and regional lymph node dose intensification.
