Study on modifiable factors of neuropsychiatric subsyndromes in patients with dementia

Abstract:Objective To analyze the modifiable factors of neuropsychiatric subsyndromes(affective symptom clusters,psychosis symptom clusters,and hyperactivity symptom clusters)in patients with dementia at home.Methods Convenience sampling method was used to investigate 193 patients with dementia at home and their caregivers in the memory clinic of the Department of Neurology of the First Affiliated Hospital of Jilin University from November 2020 to May 2022 by using Neuropsychiatric Inventory(NPI),Positive Aspects of Caregiving(PAC),Family Attitude Scale,Simplified Coping Style Questionnaire and Family Environment Scale.Results The critical dimension of emotional expression(β=0.290)and psychotropic drug use(β=0.127)were risk factors of NPI total score.The life outlook dimension of PAC(β=-0.159)was a protective factor for affective symptom clusters in patients with dementia at home.Age>70-80 years(β=0.170)and psychotropic drug use(β=0.238)were risk factors for psychosis symptom clusters.In addition,privacy and sociability in the home environment(β=-0.175)were protective factors for psychosis symptom clusters.Hostility in caregivers'expressed emotions(β=0.219)was a risk factor for hyperactivity symptom clusters.Conclusion Psychotropic drug use,PAC and expressed emotions,and home environment are modifiable factors for neuropsychiatric symptoms in patients with dementia at home.Intervention based on modifiable patient-caregiver-environment factor may be a new way to scientifically and effectively treat neuropsychiatric symptoms in patients with dementia.
