Observation on the therapeutic effect of ultrasound-guided injection combined with joint mobilization for frozen shoulder

Abstract:Objective To observe the clinical efficacy of ultrasound-guided injection of anti-inflammatory and analgesic solution combined with manual mobilization in the treatment of shoulder stiffness.Methods A total of 160 patients with shoulder stiffness treated in the Department of Rehabilitation,Dongyang People's Hospital from January 2024 to January 2025 were selected and divided into Group A and Group B using a random number table method,with 80 cases in each group.Group A received ultrasound-guided injection of anti-inflammatory and analgesic solution combined with manual mobilization,while Group B received manual mobilization alone.Both groups were treated 3 times per week for a course of 4 weeks.The Visual Analogue Scale(VAS)score for pain,active range of motion(AROM)of the shoulder,Constant-Murley Score(CMS),and modified Barthel Index(MBI),and the thickness of the axillary recess capsule on the affected side(measured by ultrasound)were compared between the two groups before treatment,and at 4 and 12 weeks after treatment.Adverse events and follow-up outcomes at 6 months post-treatment were also recorded.Results At 4 and 12 weeks post-treatment,both groups showed a significant decrease in VAS scores and axillary recess capsule thickness compared to baseline(P<0.05),and a significant increase in AROM,CMS,and MBI scores(P<0.05).Furthermore,at both time points,Group A demonstrated significantly lower VAS scores and capsule thickness(P<0.05),and significantly higher AROM,CMS,and MBI scores(P<0.05)than Group B.There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).At the 6-month follow-up,the proportion of requiring additional treatment in Group A was significantly lower than that of Group B(P<0.05).Conclusion Ultrasound-guided injection of anti-inflammatory and analgesic solution combined with manual joint mobilization is a safe and effective treatment for shoulder stiffness.It can significantly relieve pain,improve joint function and daily living activities,and reduce inflammatory thickening of the joint capsule.
