Effects of circular stapler and linear stapler on perioperative indexes,gastrointestinal function and nutritional status after radical distal gastrectomy

Abstract：Objective To explore the effects of circular stapler and linear stapler on perioperative indexes,gastrointestinal function and nutritional status after radical distal gastrectomy.Methods A retrospective collection was performed on clinical information of 124 patients with gastric cancer who underwent radical distal gastrectomy in the First Affiliated Hospital of Huzhou University from January 2022 to December 2024.According to different choices of staplers,they were divided into circular group(n=64,circular stapler)and linear group(n=60,linear stapler).The perioperative indexes,gastrointestinal function[gastrin(GAS),vasoactive intestinal peptide,cholecystokinin(CCK),and intestinal fatty acid binding protein(I-FABP)],nutritional status[transferrin(TRF),albumin,and prealbumin]before treatment and 1 month after treatment,and complications in the two groups were compared.Results The anastomosis time,first exhaust time,postoperative feeding time and length of hospital stay in circular group were longer than those in linear group,with significant differences(P＜0.05).Within each age and gender stratification,the anastomosis time,first exhaust time,postoperative feeding time and hospitalization time in circular group were longer than those in linear group,and the differences were statistically significant(P＜0.05).However,compared with different age groups of the same operation,the anastomosis time,first exhaust time,postoperative feeding time and hospitalization time of patients＞65 years old were longer than those ≤ 65 years old(P＜0.05).After surgery,the levels of GAS and CCK in circular group were lower than those in linear group,while the level of I-FABP was higher than that in linear group,with significant differences(P＜0.05).After surgery,TRF in circular group was lower than that in linear group,with significant differences(P＜0.05).The incidence of main complication(anastomotic stenosis)in circular group was significantly higher than that in linear group(14.06％vs 3.33％,P＜0.05).Conclusion Compared with circular stapler,linear stapler is more conducive to early recovery of patients after radical distal gastrectomy,improving gastrointestinal function and nutritional status,which has good promotion value.
