Clinical significance and prognostic evaluation value of E-cadherin and Ki-67 in gastric neuroendocrine tumors

Abstract:Objective To investigate the expression characteristics of E-cadherin and Ki-67 in gastric neuroendocrine tumors(GNET)and their relationship with postoperative prognosis of patients,aiming to evaluate their clinical application value as prognostic biomarkers.Methods A total of 64 patients with GNET who were admitted to Dafeng Hospital Affiliated to Nantong University Xinglin College from December 2018 to January 2020 were selected as the research objects and divided into a good prognosis group(n=24)and a poor prognosis group(n=40)according to the follow-up results at 48 months after the operation.The general clinical data of the two groups of patients were collected,mRNA expression levels of E-cadherin and Ki-67 in tumor tissues were measured using RT-PCR,and protein expression was observed via immunohistochemistry.The median expression levels of the two were grouped and compared.The ROC curve was drawn to evaluate their predictive ability for poor prognosis,and survival analysis was performed using the Kaplan-Meier method.Results The poor prognosis group exhibited significantly higher mRNA expression levels of E-cadherin and Ki-67 compared to the good prognosis group(P<0.001).Immunohistochemistry also demonstrated more intense protein expression in the poor prognosis group.A significantly greater proportion of high-expression patients was found in the poor prognosis group than in the good prognosis group(E-cadherin:70.00% vs 16.67%,P<0.001;Ki-67:65.00% vs 25.00%,P=0.002),and the difference was statistically significant.ROC analysis showed that both E-cadherin and Ki-67 had good predictive performance for prognosis of GNET(AUC=0.852 and 0.798,respectively;both P<0.001).Survival analysis revealed that high expression of both markers was associated with significantly worse outcomes(Log-rank P<0.001).Conclusion High expression of E-cadherin and Ki-67 is closely associated with poor prognosis in patients with GNET and has a high predictive value.Both are expected to serve as potential biomarkers for postoperative risk assessment of GNET,providing important references for individualized clinical treatment and precise management.
