Effect of traditional Chinese medicine fumigation and washing combined with moxibustion on levels of serum nerve factors and nerve conduction velocity in patients with blood stasis diabetic peripheral neuropathy

Abstract：Objective To study the effect of traditional Chinese medicine fumigation and washing combined with moxibustion on levels of serum nerve factors and nerve conduction velocity in patients with blood stasis diabetic peripheral neuropathy(DPN).Methods From May 2024 to May 2025,184 patients with blood stasis DPN admitted to Lishui Hospital of Traditional Chinese Medicine were selected as the research objects,4 cases were lost,and 180 cases were finally included in the study.They were divided into the Western medicine group,the moxibustion group,the traditional Chinese medicine fumigation and washing group,and the moxibustion+traditional Chinese medicine fumigation and washing group by random number table method,with 45 cases in each group.On the basis of routine treatment,the Western medicine group was only treated with Western medicine.On the basis of the treatment of the Western medicine group,the moxibustion group,the traditional Chinese medicine fumigation and washing group and the moxibustion+traditional Chinese medicine fumigation and washing group were given moxibustion at the well points of the six meridians of the lower limbs,traditional Chinese medicine fumigation and washing,moxibustion at the well points of the six meridians of the lower limbs and traditional Chinese medicine fumigation and washing.The course of treatment was 14 d.The clinical efficacy of the four groups after treatment was statistically analyzed.The traditional Chinese medicine syndrome scores,Toronto Clinical Scoring System(TCSS)scores,levels of serum nerve factors,temperature of the well points of the six meridians of the lower limbs,and nerve conduction velocity were compared among the four groups before and after treatment.Results After treatment,the total effective rate of the moxibustion+traditional Chinese medicine fumigation and washing group was higher than that of the traditional Chinese medicine fumigation and washing group,the moxibustion group and the Western medicine group,and the traditional Chinese medicine fumigation and washing group and the moxibustion group were higher than the Western medicine group(P＜0.05).After treatment,the scores of main symptoms,secondary symptoms,TCSS scores and levels of serum 5-hydroxytryptamine(5-HT)and myelin basic protein(MBP)in the four groups were lower than those before treatment.After treatment,the scores of main symptoms,secondary symptoms,TCSS scores and levels of serum 5-HT and MBP in the moxibustion+traditional Chinese medicine fumigation and washing group were lower than those in the traditional Chinese medicine fumigation and washing group,the moxibustion group,and the Western medicine group.After treatment,the scores of main symptoms,secondary symptoms,TCSS scores and levels of serum 5-HT and MBP in the traditional Chinese medicine fumigation and washing group and the moxibustion group were lower than those in the Western medicine group(P＜0.05).After treatment,the levels of serum β-endorphin(β-EP),brain-derived neurotrophic factor(BDNF),and the temperature of Lidui,Yinbai,Dadun,Zuqiaoyin,Zhiyin,Yongquan,as well as the conduction velocity of motor nerves and sensory nerves of median nerve and common peroneal nerve in the four groups were higher than those before treatment.After treatment,the levels of serum β-EP,BDNF,and the temperature of Lidui,Yinbai,Dadun,Zuqiaoyin,Zhiyin,Yongquan,as well as the conduction velocity of motor nerves and sensory nerves of median nerve and common peroneal nerve in the moxibustion+traditional Chinese medicine fumigation and washing group were higher than those in the traditional Chinese medicine fumigation and washing group,the moxibustion group and the Western medicine group.After treatment,the levels of serum β-EP,BDNF,and the temperature of Lidui,Yinbai,Dadun,Zuqiaoyin,Zhiyin,Yongquan,as well as the conduction velocity of motor nerves and sensory nerves of median nerve and common peroneal nerve in the traditional Chinese medicine fumigation and washing group and the moxibustion group were higher than those in the Western medicine group(P＜0.05).Conclusion The treatment of patients with blood stasis DPN by traditional Chinese medicine fumigation and washing,moxibustion combined with Western medicine can effectively regulate the levels of serum β-EP,5-HT,BDNF and MBP,improve their traditional Chinese medicine syndromes and clinical symptoms,and increase the temperature of the well points of the six meridians of the lower limbs and the nerve conduction velocity,which has a good clinical efficacy.
