Analysis of influence of antibiotic duration on jaw cyst with bone grafting and influencing factors of postoperative infection

Abstract:Objective To retrospectively analyze the influence of prolonging the prophylactic medication time of cefuroxime on postoperative infection in patients with jaw cyst and bone grafting,and to analyze the influencing factors of postoperative infection.Methods A total of 110 patients with jaw cyst treated in Lishui Central Hospital from January 2022 to December 2024 were retrospectively selected and divided into control group(cefuroxime was applied at 0.5 h before surgery and at 24 h after surgery)and observation group(cefuroxime was applied at 0.5 h before surgery and at 72 h after surgery)according to the duration of postoperative use of cefuroxime.The postoperative recovery time and plaque index(PLI),pain status and inflammatory factors[C-reactive protein(CRP)and white blood cell count(WBC)]before and after surgery and postoperative infection were compared between both groups.Logistic regression analysis was adopted to analyze the risk factors affecting postoperative infection of jaw cyst with bone grafting.Results The postoperative recovery time of the observation group[(50.11±4.11)d vs(55.35±4.12)d]was shorter than that of the control group,and the PLI of the observation group[(0.98±0.11)vs(1.11±0.12)]was shorter than that of the control group(P<0.05).The pain score of the observation group 5 d after surgery[(1.43±0.21)points vs(1.86±0.23)points]was lower than that of the control group(P<0.05).The levels of CRP[(5.33±1.04)μg/L vs(7.01±1.03)μg/L]and WBC[(6.53±1.31)× 109/L vs(8.14±1.32)× 109/L]in the observation group after surgery were lower(P<0.05).The postoperative infection rate in the observation group(3.64%)was lower than that in the control group(16.36%)(P<0.05).Logistic regression analysis indicated that the duration of postoperative use of cefuroxime and surgical time were independent factors affecting postoperative infection in patients with jaw cyst and bone grafting(P<0.05).Conclusion Prolonging the prophylactic medication time of cefuroxime can reduce postoperative infection in patients with jaw cyst and bone grafting,shorten cure time,reduce PLI,relieve postoperative pain,and reduce the levels of inflammatory factors.Duration of postoperative use of cefuroxime and surgical time are the factors affecting postoperative infection in patients with jaw cyst and bone grafting.
