Clinical efficacy analysis of percutaneous minimally invasive resection of intraspinal schwannoma

Abstract:Objective To compare the clinical efficacy of percutaneous minimally invasive surgery and traditional open surgery for the resection of intraspinal schwannoma.Methods From January 2022 to June 2023,a retrospective analysis was conducted on 12 patients who underwent percutaneous minimally invasive paravertebral approach for resection of intraspinal subdural schwannoma(minimally invasive channel group)in the Department of Neurosurgery,Tangdu Hospital,Air Force Medical University.During the same period,13 patients who underwent traditional posterior total laminectomy for intraspinal subdural schwannoma(traditional open group)were used as controls.All patients were followed up for more than 6 months,and the differences in maximum tumor diameter,surgical incision length,intraoperative blood loss,operative duration,postoperative hospital stay,and postoperative complications between the two groups of patients were compared.Results The general data(gender,age,tumor location,and preoperative symptoms)between the two groups showed no significant differences and were comparable.The maximum tumor diameter in the minimally invasive channel group was(2.52±0.83)cm,which was smaller than(3.20±0.85)cm in the traditional open group.However,the difference between the two groups was not statistically significant(P=0.057).In the minimally invasive channel group,the surgical incision length was(2.46±0.44)cm,the intraoperative blood loss was(81.25±35.93)mL,the operative duration was(131.25±23.85)min,the postoperative 24 h drainage volume was(60.00±14.46)mL,the postoperative 3 d Visual Analogue Scale score was(2.08±0.67),the postoperative ground movement time was(2.00±0.85)d,and the postoperative hospital stay was(6.33±0.65)d,which were significantly lower than those in the traditional open group(P<0.01),and no serious postoperative complications occurred in both groups of patients.Conclusion Percutaneous minimally invasive approach for microsurgical resection of intraspinal schwannoma through paravertebral approach has the characteristics of minimal trauma,fast recovery,and good spinal stability,achieving the goal of"minimally invasive surgery".Compared with traditional open surgery,it has obvious advantages,but attention should be paid to selecting appropriate cases.
