Safety and efficacy of robot-assisted laparoscopic appendiceal flap in the treatment of complex ureteral strictures

Abstract:Objective To evaluate the safety and efficacy of robot-assisted laparoscopic appendiceal flap repair for complex ureteral strictures(US).Methods A retrospective analysis was conducted on 7 patients with right US treated at Tangdu Hospital,Air Force Medical University from October 2022 to June 2024.The mean age was(39.43±11.27)years.Etiologies included postoperative ureteral stones(4 cases),surgical injury(2 cases),and post-pyeloplasty stricture(1 case).All patients had a history of multiple prior interventions,including balloon dilation and repeated ureteral stent placements.Stricture locations were mid-ureter(3 cases),upper ureter(3 cases),and combined mid-upper ureter(1 case),with a mean stricture length of(4.79±1.35)cm.Robot-assisted laparoscopic appendiceal flap reconstruction was performed.Parameters recorded included operative time,intraoperative blood loss,postoperative hospital stay,intra-/postoperative complications,and follow-up outcomes.Results All 7 procedures were successfully performed without intraoperative complications.Mean operative time was(167.86±17.99)min,estimated blood loss was(117.14±49.57)mL,and mean hospital stay was(7.29±1.11)d.No high-grade complications occurred.During a mean follow-up of(24.57±10.92)months,the surgical success rate was 100%.Conclusion Robot-assisted laparoscopic appendiceal flap reconstruction is a safe and effective technique for repairing long-segment mid-to-upper right US.
