Influencing factors of functional dyspepsia in children and changes in expression levels of serum CGRP,ghrelin,PRDX1 and GLP-1

Abstract:Objective To explore the influencing factors of functional dyspepsia(FD)in children and the changes in expression levels of serum calcitonin gene-related peptide(CGRP),ghrelin,peroxiredoxin 1(PRDX1),and glucagon-like peptide-1(GLP-1).Methods A total of 134 children with FD admitted to Huzhou Maternity and Child Health Care Hospital from November 2022 to November 2024 were selected and included in the FD group,and another 134 healthy children who had physical examinations were selected as the control group according to a 1∶1 ratio.The clinical data and expression levels of serum CGRP,ghrelin,PRDX1,and GLP-1 of the FD group and the control group were compared.Multivariate logistic regression analysis was used to analyze the influencing factors of FD in children.The children with FD were classified as positive,and the healthy children who had physical examinations were classified as negative.ROC curves were drawn to analyze the diagnostic value of single and combined detection of serum indicators for FD in children.Results Compared with the control group,the FD group had a higher proportion of children aged>9 years,mothers with an education level below undergraduate,learning difficulties,frequent snack intake,a family history of upper gastrointestinal diseases,and Helicobacter pylori infection(P<0.05).Compared with the control group,the expression levels of serum CGRP and PRDX1 were higher in the FD group,and the expression levels of serum ghrelin and GLP-1 were lower in the FD group(P<0.05).The results of multivariate logistic regression analysis showed that children aged>9 years(OR=1.929,95%CI=1.042-3.570),mothers with an education level below undergraduate(OR=2.147,95%CI=1.262-3.652),frequent snack intake(OR=1.996,95%CI=1.269-3.139),a family history of upper gastrointestinal diseases(OR=1.826,95%CI=1.154-2.888),Helicobacter pylori infection(OR=2.022,95%CI=1.132-3.612),increased expression level of serum CGRP(OR=1.910,95%CI=1.275-2.860)and increased expression level of serum PRDX1(OR=1.919,95%CI=1.146-3.214)were risk factors for the occurrence of FD in children(P<0.05),and increased expression level of serum ghrelin(OR=0.476,95%CI=0.268-0.843)and increased expression level of serum GLP-1(OR=0.498,95%CI=0.321-0.770)were protective factors for the occurrence of FD in children(P<0.05).The children with FD were classified as positive,and the healthy children who had physical examinations were classified as negative.ROC curves were drawn to analyze the diagnostic value of single and combined detection of CGRP,ghrelin,PRDX1,and GLP-1 for the occurrence of FD in children.The AUC values were 0.819,0.808,0.834,0.751,and 0.933,respectively,and the combined detection had the highest value(Z=3.908,4.036,3.460,and 5.392,P<0.05),with a sensitivity of 88.81%and a specificity of 84.33%.Conclusion The influencing factors of FD in children include age>9 years,mothers with an education level below undergraduate,frequent snack intake,a family history of upper gastrointestinal diseases,Helicobacter pylori infection,increased expression levels of serum CGRP and PRDX1,and decreased expression levels of serum ghrelin and GLP-1.The combined detection of changes in expression levels of serum CGRP,ghrelin,PRDX1,and GLP-1 has a high diagnostic value for FD in children and can be used as a new target for clinical diagnosis and treatment.
