The relationship between serum CysC,MMP-9,and HCY levels and their cardiac function in patients with chronic pulmonary heart disease and their predictive value for prognosis

Abstract:Objective To study the relationship between serum cystatin C(CysC),matrix metalloproteinase-9(MMP-9)and homocysteine(HCY)levels and their cardiac function in patients with chronic pulmonary heart disease and their predictive value for prognosis,so as to provide reference and basis for prognosis evaluation of patients with chronic pulmonary heart disease.Methods The clinical data of 173 patients with chronic pulmonary heart disease who were treated in Jinhua Hospital Affiliated to Zhejiang University School of Medicine from January 2021 to September 2023 and followed up for one year after discharge were collected retrospectively and taken as the case group.The physical examination data of 179 healthy people who underwent physical examination in the hospital during the same period were selected as the control group.The general data and levels of serum CysC,MMP-9,and HCY and myocardial injury indexes of the two groups were statistically analyzed.According to the cardiac function of patients with chronic pulmonary heart disease and the prognosis during follow-up,they were divided into grade Ⅱ group(49 cases),grade Ⅲ group(72 cases),grade Ⅳ group(52 cases),survival group(147 cases),and death group(26 cases).The levels of serum CysC,MMP-9,and HCY and myocardial injury indexes of patients with chronic pulmonary heart disease with different cardiac function grades and different prognoses were compared.The correlation between serum CysC,MMP-9,HCY and myocardial injury in patients with chronic pulmonary heart disease and its predictive value for prognosis were analyzed.Results The mean pulmonary artery pressure and levels of serum CysC,MMP-9,HCY,NT-proBNP,cTN I,and CK in the case group were higher than those in the control group(P<0.05).The levels of serum CysC,MMP-9,HCY,NT-proBNP,cTN I,and CK in the grade Ⅱ group,grade Ⅲ group and grade Ⅳ group showed an increasing trend(P<0.05).The levels of serum CysC,MMP-9,HCY,NT-proBNP,cTN Ⅰ,and CK in the death group were higher than those in the survival group(P<0.05).There was a significant positive correlation between levels of serum CysC,MMP-9,and HCY and levels of serum NT-proBNP,cTN I,and CK in patients with chronic pulmonary heart disease(r=0.463,0.619,0.476,0.502,0.544,0.459,0.488,0.621,0.522,P<0.05).The area under the curve of serum CysC,MMP-9,and HCY in predicting the prognosis of patients with chronic pulmonary heart disease was higher than that of the three alone(P<0.05),and the sensitivity and specificity were 73.08%and 85.03%,respectively.Conclusion The occurrence and development of chronic pulmonary heart disease can cause increased levels of serum CysC,MMP-9,and HCY and myocardial injury.There is a significant correlation between levels of serum CysC,MMP-9,and HCY and myocardial injury indexes,and the combination of the three can help to improve the predictive value for the prognosis of patients with chronic pulmonary heart disease.
