Correlation analysis of glucose and lipid metabolism,sex hormone levels,and lumbar bone mineral density in perimenopausal women

Abstract:Objective To analyze the correlation of glucose and lipid metabolism,sex hormone levels,and lumbar bone mineral density in perimenopausal women.Methods A total of 132 perimenopausal women who visited the Department of Perimenopausal Health Care and Gynecological Endocrinology in Shaoxing University Affiliated Maternal and Child Health Hospital from March 2024 to March 2025 were selected as the study subjects.Their general demographic data,glucose and lipid metabolism indicators,sex hormone levels,and bone mineral density-related indicators were collected.Pearson's correlation analysis was used to explore the correlation of glucose and lipid metabolism,sex hormone levels,and lumbar bone mineral density.According to the T-value of lumber bone mineral density,women enrolled were divided into the abnormal bone mass(osteopenia and osteoporosis)group(n=40)and the normal bone mass group(n=92).Univariate and multivariate logistic analyses were used to identify the influencing factors of abnormal lumber vertebral bone mass in perimenopausal women.Results Pearson's correlation analysis showed that triglyceride was positively correlated with femoral neck bone mineral density and hip bone mineral density(P<0.05).Low-density lipoprotein cholesterol was negatively correlated with L1 bone mineral density(P<0.05).High-density lipoprotein cholesterol was positively correlated with vitamin D(P<0.05).Lipoprotein(a)[Lp(a)]was negatively correlated with L1 bone mineral density,L2 bone mineral density,L3 bone mineral density,L4 bone mineral density,L1-L4 bone mineral density,femoral neck bone mineral density and hip bone mineral density(P<0.05).Moreover,it was positively correlated with beta-isomerized C-terminal telopeptide of type 1 collagen(β-CTx),alkaline phosphatase(ALP),and N-terminal mid-fragment of osteocalcin(N-MID)(P<0.01).Fasting plasma glucose was negatively correlated with L1 bone mineral density and vitamin D(P<0.05).Pearson's correlation analysis showed that follicle-stimulating hormone was positively correlated with N-MID,total procollagen type 1 N-terminal propeptide(T-P1NP),β-CTx,and ALP(P<0.01).Luteinizing hormone(LH)was positively correlated with β-CTx and ALP(P<0.01).Estradiol was positively correlated with L2 bone mineral density,L3 bone mineral density,and L1-L4 bone mineral density(P<0.05),and negatively correlated with N-MID and ALP(P<0.05).T was positively correlated with L2 bone mineral density,L3 bone mineral density,L4 bone mineral density,and L1-L4 bone mineral density(P<0.05).Dehydroepiandrosterone(DHEA)was positively correlated with L1 bone mineral density,L2 bone mineral density,L1-L4 bone mineral density,and vitamin D(P<0.05),and was negatively correlated with β-CTx(P<0.05).Multivariate logistic analysis found that β-CTx and Lp(a)were independent risk factors for abnormal bone mass in perimenopausal women(P<0.01),while LH,DHEA,T-P1NP,and vitamin D were protective factors(P<0.05).Conclusion The lumbar bone mineral density in perimenopausal women is affected by both glucose and lipid metabolism disorders and changes in sex hormone levels.Therefore,comprehensive intervention should be carried out to maintain bone health.
