Diagnostic value of serum miR-155,miR-140,and miR-146a in knee osteoarthritis and their relationship with disease severity

Abstract：Objective To explore the diagnostic value of serum micro-ribonucleic acid-155(miR-155),micro-ribonucleic acid-140(miR-140),and micro-ribonucleic acid-146a(miR-146a)in knee osteoarthritis(KOA)and their relationship with disease severity.Methods A total of 87 patients with KOA admitted to Huzhou Hospital of Traditional Chinese Medicine from May 2023 to January 2025 were selected as the observation group and classified into grade Ⅱ group(29 cases),grade Ⅲ group(33 cases),and grade Ⅳ group(25 cases)according to the Kellgren-Lawrence classification system.Another 87 healthy individuals were selected as the control group at a 1∶1 ratio during the same period.The levels of serum miR-155,miR-140,and miR-146a in the observation group and the control group,as well as in patients with different grades of KOA were compared.KOA was classified as positive and healthy individuals as negative.Receiver operating characteristic(ROC)curves were drawn to analyze the diagnostic value of serum miR-155,miR-140,and miR-146a alone and in combination for the occurrence of KOA.Spearman correlation analysis was used to analyze the correlation.Results Compared with the control group,the levels of serum miR-155 and miR-146a were higher,and the level of serum miR-140 was lower in the observation group(P＜0.001).Compared with the grade Ⅱ and grade Ⅲ groups,the levels of serum miR-155 and miR-146a were higher,and the level of serum miR-140 was lower in the grade Ⅳ group(P＜0.05).Compared with the grade Ⅱ group,the levels of serum miR-155 and miR-146a were higher,and the level of serum miR-140 was lower in the grade Ⅲ group(P＜0.05).KOA was classified as positive and healthy individuals as negative.ROC curves were drawn to analyze the diagnostic value of miR-155,miR-140,and miR-146a alone and in combination for the occurrence of KOA.The area under the curve values were 0.795,0.812,0.761,and 0.921,respectively.The combined detection had the highest value(P＜0.001),with a sensitivity of 86.21％and a specificity of 89.66％.The progression of KOA was positively correlated with the levels of serum miR-155 and miR-146a and negatively correlated with the level of serum miR-140(P＜0.001).Conclusion The levels of serum miR-155 and miR-146a are higher,and the level of serum miR-140 is lower in patients with KOA.Their expressions are closely related to the severity of the disease.The combined detection of the three has high diagnostic value for KOA and can be used as a new target for clinical diagnosis and treatment.
