Study on the relationship between serum TFF1,Smad4,CCL22 and TPS levels and clinicopathological features and prognosis of patients with oral cancer

Abstract:Objective To investigate the relationship between the serum levels of trefoil factor 1(TFF1),mothers against decapentaplegic homolog 4(Smad4),CC chemokine ligand 22(CCL22)and tissue polypeptide specific antigen(TPS)and the clinicopathological features and prognosis of patients with oral cancer.Methods A total of 153 patients with oral cancer admitted to Ningbo Medical Center Li Huili Hospital from January 2020 to October 2022 were selected as the oral cancer group.All patients with oral cancer underwent surgical treatment.The starting point of observation was discharge after operation,and the end point of observation was disease diagnosis to the last follow-up or patient death.The patients and their families were followed up by telephone and outpatient follow-up.The follow-up deadline was October 2024,and no one was lost.Among them,41 cases with poor prognosis(recurrence or death)during the follow-up period were regarded as the poor prognosis group,and the remaining 112 cases as the good prognosis group.Additionally,153 volunteers with normal physical examination during the same period were selected as the healthy control group.The clinical data,serum levels of TFF1,Smad4,CCL22,and TPS,and serum levels of TFF1,Smad4,CCL22,and TPS in patients with oral cancer with different pathological characteristics and prognosis between oral cancer group and healthy control group were compared.Receiver operating characteristic(ROC)curve was drawn,the area under the curve(AUC)was obtained,and the predictive value of single and combined detection of serum TFF1,Smad4,CCL22 and TPS for poor prognosis of patients with oral cancer was analyzed.Results Compared with the healthy control group,the serum levels of TFF1,CCL22 and TPS in the oral cancer group were higher,while the serum level of Smad4 was lower(P<0.001).Compared with the patients with well-differentiated oral cancer,with clinical stages Ⅰ-Ⅱ and no lymph node metastasis,the patients with moderately and poorly differentiated oral cancer,with clinical stages Ⅲ-Ⅳ and lymph node metastasis had higher serum levels of TFF1,CCL22 and TPS,but lower serum level of Smad4(P<0.01).Compared with the good prognosis group,the serum levels of TFF1,CCL22 and TPS in the poor prognosis group were higher,while the serum level of Smad4 was lower(P<0.001).The poor prognosis group was included as positive,and the good prognosis group was included as negative.ROC analysis showed that the AUC value of the combined detection of serum TFF1,Smad4,CCL22 and TPS in predicting the poor prognosis of patients with oral cancer was 0.924,which was higher than that of single detection of each index(0.718,0.732,0.734 and 0.753,P<0.05).Conclusion The serum levels of TFF1,Smad4,CCL22 and TPS in patients with oral cancer are abnormally expressed,which are closely related to the pathological grade,clinical stage and lymph node metastasis of the patients,and the combined detection of the four is more advantageous in predicting the poor prognosis of patients with oral cancer.
