Construction and therapeutic effect evaluation of multidisciplinary team diagnosis and treatment model for stress urinary incontinence based on holistic integrative medicine theory

Abstract:Objective To construct and evaluate the therapeutic effect of multidisciplinary team(MDT)diagnosis and treatment model in patients with stress urinary incontinence(SUI)based on holistic integrative medicine(HIM)theory.Methods A total of 140 female SUI patients admitted to the Center for Pelvic Floor Medicine of the Affiliated Hospital of Fujian University of Traditional Chinese Medicine from October 2019 to July 2021 were selected and randomly divided into MDT diagnosis and treatment group and conventional diagnosis and treatment group by computer randomization method,with 70 cases in each group.MDT diagnosis and treatment group was given MDT and full-cycle diagnosis and treatment model,and conventional diagnosis and treatment group was given conventional diagnosis and treatment model.Each group received 1 course of intervention.The improvement rate of urinary incontinence and the change of surface electromyogram(EMG)value of pelvic floor muscle were observed before the intervention and after 1 course of intervention.International Consultation on Incontinence Questionnaire Short Form(ICI-Q-SF)score and the satisfaction of diagnosis and treatment service were observed before the intervention,after 1 course of intervention and 3 months after 1 course of intervention.Results The clinical improvement rate of the MDT diagnosis and treatment group(92.9%)was significantly higher than that of the conventional diagnosis and treatment group(87.1%),and the difference was statistically significant(P<0.05).Before the intervention,there was no significant difference between the two groups in the surface EMG value of pelvic floor muscle(rapid contraction stage,tension contraction stage and endurance contraction stage).After 1 course of intervention,the surface EMG value of pelvic floor muscle in the MDT diagnosis and treatment group was higher than that in the conventional diagnosis and treatment group(P<0.05).Before intervention,after 1 course of intervention and 3 months after 1 course of intervention,the inter-group,time and interaction of ICI-Q-SF in the two groups were statistically significant(Pinar-group<0.05,Ptime<0.01,Pinteraction<0.01).Before intervention,there was no significant difference in ICI-Q-SF scores between the two groups.After 1 course of intervention,the ICI-Q-SF scores of the two groups showed a downward trend over time,and the downward trend was more obvious in the MDT diagnosis and treatment group.After 1 course of intervention,the satisfaction rate of MDT diagnosis and treatment group(95.7%)was higher than that of conventional diagnosis and treatment group(90.0%).Conclusion The application of MDT diagnosis and treatment model based on HIM to SUI can improve the treatment process,optimize the treatment plan,enhance the therapeutic effect of SUI,enhance the satisfaction of patients with the overall medical service,and improve the long-term curative effect.
