The differential diagnostic value of DCE-MRI in plasma cell mastitis and non-mass-like invasive breast cancer

Abstract:Objective To analyze the differential diagnostic value of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)in plasma cell mastitis(PCM)and non-mass-like invasive breast cancer(IBC-NML).Methods Thirty patients with PCM(PCM group)and thirty patients with IBC-NML(IBC-NML group)who underwent DCE-MRI were retrospectively ana-lyzed.MRI features,including involvement of the areolar region,distribution pattern,dendritic dilatation of breast ducts,inter-nal enhancement pattern,degree of enhancement in the first phase,apparent diffusion coefficient(ADC)value,and time-signal intensity curve(TIC)type,were compared between the two groups.The multiple Logistic regression analysis was used to screen independent predictive factors that could distinguish PCM and IBC-NML.ROC curves were drawn to analyze the diagnostic effi-cacy of single and combined MRI features in differentiating PCM from IBC-NML.Results Significant differences were ob-served between the two groups in terms of areolar involvement,distribution pattern,ductal dendritic dilatation,internal enhance-ment pattern,degree of enhancement in the first phase,and ADC value(all P<0.05).The areas under the curve(AUC)for single and combined MRI features including areolar involvement,multi-regional distribution,ductal dendritic dilatation,mul-tiple annular nodules,mild enhancement in the first phase,and ADC value in differentiating PCM from IBC-NML were 0.700,0.667,0.800,0.817,0.650,0.690,and 0.912,respectively.The combined diagnostic model showed a significantly higher AUC than any single MRI feature(all P<0.05).Conclusion Areolar involvement,multi-regional distribution,ductal dendritic dilatation,multiple annular nodules,mild enhancement in the first phase,and ADC value contribute to the differentiation be-tween PCM and IBC-NML.The combination of multiple MRI features can improve diagnostic performance.
